Crestwood Care Center has now become Symphony of Crestwood and is a proud member of the Symphony Post Acute Network.

Along with this new name comes a new philosophy of care and a complete upgrade and renovation to the facility. Symphony of Crestwood, 14255 Cicero Ave., Crestwood, will focus on providing short-term post acute nursing and rehabilitation to the surrounding community. An open house will be scheduled in the spring of 2013.

Symphony of Crestwood works closely with top physicians, nurses and therapists and our newly renovated, modern facility provides an environment filled with amenities to facilitate the quickest recovery with optimal results. Symphony of Crestwood’s specialty programs include the S.T.A.T Program, developed to prevent unnecessary hospital readmissions, the Heart for Life Cardiac Care Program, In-House Hemodialysis, Orthopedic Rehabilitation, Complex Wound Care and Pulmonary Care.
